Suggest Remedies For Urinary Retention Post Catheter Insertion

A patient for laparoscopic appendectomy had a pre-op urinary catheter inserted (not sure why it was necessary) and subsequently developed urinary retention and was catheterized a second time. Was told retention resulted form allergic reaction to latex in the pre-op foley catheter. Patient exhibited no systemic symptoms and has been previously exposed to latex without incident. Does this sound likely/reasonable?

General Surgeon 's  Response
Hi,

Noted the history about pre-op urethral catheter for laparoscopic appendectomy, got urinary retention after removal of catheter and need to re-insert the catheter.
To answer your specific answers first:
Pre-op catheter in urinary bladder is necessary as the appendix is in lower abdomen and a bull bladder during surgery can cause problems for the procedure to be performed, hence it is necessary and nothing wrong into it.
Secondly latex can cause some sort of retention anytime during life as many surgeons develop allergy to latex gloves and many people are known to cause allergy to condom latex.
There are other reasons too like urethral infection, inflammation or congestion of prostate and so on.
Hence I would advise you the following in such a situation:
First of all consult your operating Surgeon to see for the cause, get a per-rectal digital palpation of prostate and get an ultrasonography of abdomen for urinary system.
Send the urine for microscopy, culture and sensitivity tests.
Wait to see the result after catheter removal.
If needed you can certainly ask for a second opinion of a Urologist to ascertain the cause and get treated accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant, ENT & General Surgeon
