Hi,
For fluctuating level of sugar,
Insulin pump is good option it administers proper amount of insulin,

Regarding your dose of medication your taking you will have to discuss it with your treating doctor,
When blood sugar get low,you will get symptoms like light headeadness, guidiness, blackouts,
You will have to get well aware with this symptoms and discuss in details this symptoms with your treating doctor so that next time you experiences it,
You should be able to manage it by taking some food.

You can also use various portable gluco check monitor available in market,
And have a good minute ring of blood glucose at different day time,

You will have to discuss in detail with your treating doctor and make yourself aware how to counter the fluctuating glucose level.
