Hello!

Welcome on Healthcaremagic!

It seems that you are having a recurrent fungal infection. For this reason, oral therapy besides local ointment may be needed.
But, before starting therapy, it is necessary performing a bacterial culture of the material on those patches, in order to identify the fungal agent and the proper treatment options (itraconazole may not be effective any more).

Hope you will find this answer helpful!
Kind regards, Dr. Aida
