Hi... gaping of episiotomy wound mostly happen due to some sort of infections. First do a swab culture sensitivity from that area to detect any such infection. After culture report start proper medication and sometimes with only this the wound start healing and it will heal over a time period. If the wound is really a large one the it needs to be resutured but after a antibiotic coverage to take care of any active infection. If it is resutured without taking care of infection there is chances it will again open up.
