I've been having a tightness in my chest lately. I'm a smoker and sometime's it's hard to take a deep breath. What should I do and what could be the cause? There's no real pain and I'm experiencing no real difficultly breathing. Just a tightness in my chest slightly superior to the navel extending upward to a few inches inferior to the nipples. I've been diagnosed with GAD in the passed but have no previous trouble in this area. So...what could be the culprit

The initial work up includes a medical history performed by a doctor, an electrocardiogram, and a chest x-ray. Generalized anxiety disorder makes you more prone to panic attacks which can present with chest pain like this, but the other more serious causes should be ruled out first before blaming it on anxiety.
