Suggest Therapy For Primary Nocturnal Enuresis

My daughter is 6 years, 4 months. She is at a healthy weight/height and was recently at her annual well child visit with no issues. She suffers from primary nocturnal enuresis. This is not suprising to me as her grandmother, father and older sister all also suffered from the same condition. She does not follow the same pattern as her older sister. She instead wets several time in the night about every 3 hours. This is not the regular pattern of her daytime voids which can be more than 6 hours apart. We have been trying a bedwetting alarm that was very successful with my older child but it has not made much difference to this child. She seems so disoriented when she is awakened by the alarm and has not yet made it to the bathroom. We have been using the device for about a month now and have not seen any improvement. I am concerned not so much for the bedwetting but because of the frequency of voiding involuntarily at night, particularly because she seems to have the capcity and ability to hold urine much longer during the day. She is the one driving this desire to be dry at night and wear normal underwear and I want to support her desire to be dry. I am aware that she will eventually mature into the dryness but at this time, she is upset that she still wets and she really wants to be normal . I have read much material on the subject and have tried most traditional methods of restricting fluids, waking her fully, lifting and such. I have not tried pharmeceutical methods at this point. Is there any more insight or adivce you can shed on this topic.

Hello,

Bed wetting with nocturnal enuresis can be of multiple factor origin as - psychological stressor induced such as the birth of a sibling, school environment or else
- hormone level induced with ADH or hypothalamus level issues
- idiopathic (without reason)

In my clinic, I do recommend conventional measures as already tried by you and if later there are persistent symptoms. Desmopressin therapy with intranasal spray must be considered for symptomatic control.
