Suggest Treatment For GERD And Dry Cough

I have Gerd/Hiatal Hernia and Barretts and persistent dry cough. I have been waking up a few hours after going to sleep with a throbbing pain in my upper back, left side. Stops upon sitting up. Usually happens a few hours after going to sleep and getting up, walking aroudn resolves it. Then may happen again a few hours later. Could this be an esophageal spasm? I have tried all of the lifestyle recommendations.

General Surgeon 's  Response
Hi,

Pain in the back on lying down in a known case of hiatal hernia, GERD and Barrette's can be due to esophagitis caused or enhanced by the acid or stomach content reflux from stomach to the esophagus as the lower esophageal sphincter seemingly is very lax.

I would advise you the following in such a situation:
- IF the medications of PPI and Domperidone along with antacids is not helping you at all, you may be the best candidate for surgical correction of the hiatal hernia.
- Please consult a Gastro Surgeon specialized or experienced in hiatal hernia repair and go for surgery if he thinks so.
- Carry on the lifestyle changes as before.
- Sleep in a head up position to avoid reflux.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant
