PELVA is '
pityriasis lichenoides et varioliformis acuta'. It is a
rare cutaneous disorder of unknown etiology.
The cause is not yet known but 3 major theories exist:
1. An inflammatory reaction triggered by infectious agents
2. A relatively benign form of T-cell lymphoproliferative disorder
3. An immune-complex-mediated
hypersensitivity vasculitis
Treatment for this disorder include:
1. Local wound care - Infected lesions are to be treated with topical Mupirocin cream and sterile dressing changes twice daily.
2. Topical steroids can be applied to reduce irritation.
3. Topical immunomodulator crewams like Tacrolimus ointment to be applied twice daily.
4. A course of
oral antibiotics like erythromycin or doxycycline needs to be taken.
5. Exposure to sunlight may help to resolve lesions.
6. Phototherapy with artificial ultraviolet
radiation treatment using UVB or PUVA can be used as a second line therapy.
