Hi, thanks for asking from HCM.

I can understand your concern. The big blow to your head might have caused disc herniation at C4 level. But rest symptoms and findings are not related to that.

Herniated C4 disc is the cause behind numbness and headache (if occipital region). Cervical disc herniation which is compressing cord, is causing cervical myelopathy. It is a serious problem. You should

- Avoid pillow use when lying supine.
- Avoid weight lifting
- Do neck exercise under supervision.
- You can take painkiller like Gabapnetine/Nortriptyline after prescription.
- If severe vertigo, numbness, use cervical soft collar for few days.
- If still no relief, consult spine surgeon for further management.

The blurry vision could be due to headache, decreased visual acuity. You should consult ophthalmologist to rule out refractive error. It can be treated by numbered glasses.

The MRI brain findings (Punctate foci near periventricular area, subcortical white matter) are suggesting ischemic changes. These findings are mostly age related changes. In a diabetic or hypertensive patients, these changes occur early. You should follow few tips to delay further damage

- Avoid fatty and sugary food.
- Eat green vegetables, fruits
- Keep your BP under control. You should regularly measure your BP.
- Salt restricted diet (140/90mm Hg)
- You can start tablet Ecospirin after proper prescription. It will prevent further thromboembolic phenomenon.

For mucus retention cysts, you should consult ENT surgeon. He might advise you surgical excision of cyst depending upon the size.

Hope it will help. If still having doubts, do let me know.
Thanks. Take care.
