The appearance of abscess / pus coming out of the orchitis is called a sinus. Your husband has already taken treatment , is still under antibiotic coverage.
I would advise you the following: MRI and MRI sinogram of the tract. Consult a General Surgeon or a Urologist and if the medical treatment fails, your husband should undergo excision of the tract and the biopsy be done to rule out tuberculosis. IF TB is present go for a long complete course of anti-tubercular medicines.
