Hi, My son is 8 and he was recently diagnosed with acute pancreatitis. Pancreas was normal in ultrasound and MRCP. His lipase levels increased to 3 times the normal range. His pain subsides on IV fluids for a day. He has been on TPN for the past week. No pain and could tolerate liquids. He tried simple solids for a day and tolerated. The next morning he had 2 slices of gluten free toast with no butter and got backache 30 min after he ate. He has been hospitalized twice in the last 2 weeks for the same episode. His pain is triggering with food even after resting his stomach for a week. The GI specialist couldn t find any source other than an elevated igG4 of 197. He is being treated with 40mg of Prednisone for the past 3 days. Yet, he still had 2 flares of backache today that lasted for 30-45 min. This was even without eating food. Any ideas, insights would be helpful. He is a healthy child with no prior medical conditions.

Generally, acute pancreatitis is managed medically. The patient is to be hospitalised as he is having repeated episodes of pain. The treatment is to stop the oral intake of food to give rest to pancreas, analgesics, antibiotics, IV fluids.
