Suggest Treatment For Back Ache

Hi I am a 41 year old man who bulged a couple of lumbar discs 15-20 yrs ago at work. just recently approx 4-6 weeks i had a fall at the same workplace and landed fair on my bum. since then my back is killing me. I have had CT and MRI and bone scans which show DDD and annulis tears and both levels. the pain I am getting is lower back but also groin type pain. A sick pain that doesnt ease.I have a strong/high pain theshhold and dont like opiates at all. I had cabbages 3 years ago and was back at work in 3-4 weeks. I am not off work and have no intention of having time off. i have seen a Neuro Surg who has recommended a fusion l5-s1 and replascemnt l4-5. I can function at present but not at the level i want to. I coach footy, ply golf and have three full on kids and all of these are suffering. What shoulf I do?
