Hi. Sorry to hear about your problem. Although a little more information is needed to reach to a diagnosis. But lets work out some thing with the info available.
Bleeding from the penis may be due to
1. Trauma. After a trauma to a lower abdominal region or
blunt trauma to abdomen it is quite possible to have blood in the urine due damage to the kidneys/ureter/bladder/penis. In this case a strict
bed rest along with hospital admission is required.
2. stones. Stones in the
urinary tract may cause the presence of blood in the urine and additionally there may be symptoms of pain in flanks, past history of stones etc. They may need to be removed by
lithotripsy or surgically.
3. Bleeding disorders. Due to defects of clotting system in the blood, bleeding from the penis (either with urine or without urine) can be present. This option seems less likely as there is history of passage of clots some times as stated by you.

Tumor. In a patient in older age group (especially after 50years of age), any bleeding (especially painless) from penis should be an alarming sign and should not be ignored.

urologist as soon as possible and get youself evaluated thoroughly to rule out any serious illness or to manage it at an earlier stage of illness.
