Neurologist, Surgical 's  Response
Hello. Thanks for asking from HCM.

I can understand your concern. It is very sad that your grandfather had got such severe head injury. You should admit him under neurosurgery with ICU facility. He and his CT scan will be evaluated.

Treatment can be either surgically or medical.

Surgery is done to evacuate clot from brain and relieve pressure. It is indicated in case of
: Large blood clot causing pressure in brain (usually more than 30cc)
: Swelling in brain with pressure in brain
: Continuously decreasing consciousness of patient in hospital
Sometimes surgery is not done if patient is not fit for surgery due to old age, chest or heart problems. Final decision is taken by treating neurosurgeon.

Conservative or medical treatment is indicated in case of
: small blood clot
: Stable patient
: Patient is unfit for surgery
Patient is regularly evaluated by CT head and nervous system examination. In case of deterioration, patient may undergo surgery.

Hope it will help you. Thanks.

Take care.
