Suggest Treatment For Chronic Anal Fissure And Painful Bowel Movement

I am 37 year old male, I am suffering from anal fissure for the last 10 months. It started around August 2014, for a month I was managing with hi fiber diet. Since pain was more I went to Gastronologist and he suggested PEGMOVE(Poly Ethylene Glycol) and Diltiact ointment for 15 days. I was ok while taking this medicine, again fissure started when I stopped it after 15 days. I went to doctor again, he suggested to continue the PEGMOVE and smuth ointment for two months.When I stopped the medicine after two month the fissure started again. So last 8 months I am taking PEGMOVE powder to avoid pain while bowel movement. During this time I got a another issue, lot of white deposits below penis outer skin and small cracks on it. I went to urologist he suggested candid-B and anti-biotic tablets. He asked me to check the blood sugar level and it was normal. If I stop the candid ointment the problem come back again. Note : I don’t have any other problems like constipation. I used to go every day once are twice. Here are doubts 1.     Is fissure and penis fungal infection related to each other? 2.     Can I take PEGMOVE powder for long-term? 3.     Recently I visited doctor and he suggested to go for surgery for fissure. What is your advice? 4.     How do I cure issues which I face?

Hello,

As per my clinical experience, chronic anal fissure have multiple options of management. The acute condition responds to sitz bath, high fiber diet, bulk forming laxatives, analgesic and supportive care. However, if there is no response to above conservative measures, various surgical options are best measures such as:
- Lord's dilation
- Fissurectomy
- Internal sphincterotomy

Kindly discuss pros and cons of various surgical procedures in your case with your treating consultant surgeon.
