Suggest Treatment For Degenerative Bone Disease

My brother is in very poor health. He has had a multi organ transplant and has degenerative bone disease along with multiple other disease processes. The primary care physician he sees has him on Fentenal patches100mcg, what type of break tthrough pain medication would be of greater use with the Fentenal?

fentanyl patch gives opioid pain medication in a sustained release which is aborbed through the skin continuosly and goes into blood circulation giving strong pain relief for a long duration from 8-12 hours and reduces needs for oral pain medicines which are more risky for system like GI tract in feeble patients like your brother
