I am not clear about your history. I believe you stated that you have been in therapy "since 11 years" old, but it is unclear to me if you meant
psychotherapy. I would strongly recommend that you see a mental health professional to help you feel happier, think better about yourself, and think more positively. If you have been in therapy since you were 11, and are still unclear about how to "overcome these problems," then I strongly suggest you see a different therapist who can better help you and a way you can understand. You may also need to see a specialist, like a
psychiatrist, to insure that you are indeed on the proper medications for your symptoms.
Briefly, the emotion cycle is: how you think, leads to how you feel, which leads to how you act. That means that you feel better by changing the way you think, or by doing different behaviors (acting differently). But the entire cycle begins with your thinking. You have listed many problems, and you cannot overcome all of them at once. I recommend that you actively start with changing how you think about yourself. For example, you could list positive statements about how you would like to think of yourself: "I am a confident and strong person" and tell yourself such statements several times a day. You will eventually believe these statements, even if you currently do not. You need to be willing to feel differently about yourself first.
