Suggest Treatment For Distraction, Extreme Fatigue And Nightmares

I am a 46 year old female and am normally very active and fairly focussed on my work but have felt extremely distracted and lethargic and exhausted every morning when I wake up for especially the last six weeks. I also have very sometimes scary dreams and others that are very symbolic including being on war-like missions,, where I do not recognize a lot of the other warriors in them. But these dreams if they are really just dreams leave me waking up exhausted from battle in the morning. Once I know I m really awake, however I feel safe and more back to normal again and can t wait to start on with the new day. I have been on perphenazine, benztropine,and levothyroxine since I was nineteen-and-a-half years old. For the past four and a half years I have also been taking escitalipram and topiromate along with the others mentioned. I recently reduced my topiromate by 25mg as I could no longer tolerate it and I feel and do a lot better on just12.5mg of this drug. I have also reduced escitalipram back down from 10 mg to 5mg. Iam actually only supposed to be on 5mg of escitalipram to begin with but when my husband and myself were recently going through a CAS battle from them taking our newborn son away from us with no warning hours after he was born on August 16,2016 because we have both been labelled in our pasts with mental illnesses this hurt us both and caused a lot of stress and pressure on our whole family. However we still get to see him and are carrying on with our lives wll considering how upset we both were and we miss him very much. I also lost custody of my first child years ago due to my ,mental health history throughout my life. I am wondering if feeling exhausted is mostly due to stress or if I also may be on too high of a dose of perphenazine and it does have pretty severe side effects but I mostly just ignore these side effects as best as I can and have become fairly immune to them over the years. I am also wondering if the combination of that drug, my age now as well as the heat waves this summer would cause or exacerbate the symptoms of feeling exhausted and having nightmares and strange symbolic dreams every night?

The issue sounds with combined features of major depression with mood disorder. Should discuss the details of management of drug dosage with expert psychologist and counselling sessions with suggestion and behavior therapy are also cornerstones of the management in best possible manner.
