Suggest Treatment For Dizziness, Fatigue And Palp Attacks

Hello, I m fighting fatigue everyday. I was almost in tears leaving my cardio appt today because he is very happy with my heart while I feel so tired and moderately dizzy. He said the heart isn t causing dizziness. I m a clerk. At my lunch I either have to workout for 20 30 min or take 45 min nap to fight fatigue. I have arrhythmia palps vpc s that cause low bp and heart rate of 40,60,103 within 5 min timeframe. In last yr. and a half I have increased palps after eating anything or trying to sleep on left side. D dimer was negative in May but this mo is 535 twice taken a wk apart. I had an ischemic stroke in 2013 in head with no known cause. I had tests done for clot disorders at a Houston hospital. I was a couple wks pregnant. Just a minor mitral with lil regurgitation. The fatigue was minimal back then. I gained 30 lb in 2014 and my hair thinned out. I have never gained and have trouble losing any of it. I have minor migraines for yrs. I had non invasive cervical cancer fall 2015 and hysterectomy Dec. 2015. Pulmonary E in Jan 2016. Xarelto until March. D dimer then was 100 s and hemo stopped thinner. The only Med I take is aspirin 81 and propranolol 10mg as needed for palp attacks. Today cardio said take 1 before bed but there are no other meds I can take. I m seeing the electro cardio in August and another for second opinion. Electro Last yr said ablation might help but it s a hit and miss. My mom has auto immune hepatitis that showed up in her fifties. I m 38. 153 lb 5 6. My rhemotologist last yr says I have no signs of clot disorders or anything for now. I have high ANA but he said most can. I had liver and thyroid checked this yr. all fine. I m now searching for my 3rd internist because I m dizzy esp when I look down. Last Mri was Jan. All good. I haven t seen my neuro in over a yr and he isn t caught up. I m started to think there may be something wrong that s causing increase in palps. What kind of dr should I go to?

General Surgeon 's  Response
Hello,

One possibility is heart rhythm disoeders such as SVPT--Supraventricular paroxysmal tachycardia. You should check your pulse and EKG? Another possibility is MVP or mitral valve prolapse (common in 40% of people) --it can lead to similar symptoms.

One way to abort these attacks is to pinch your nostrils and blow hard through your mouth. This is called Valsalva's manouevre.

Alternately feel the carotid artery inside the sternocleido mastoid muscle in your neck--(any doctor can show you) on any one side of neck and gently massage.

This will also abort the attack. But never do it on both sides simultaneously. Personally I do not think they are panic attacks considering your personality.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Iven Romic Rommstein
