Suggest Treatment For Enlarged Bruised Scrotum After Hydrocele Repair

Surgery was done to correct a large hydrocele 2 weeks ago by a urologist. He noted the area was very inflamed during surgery. The entire scrotum was enlarged and severely bruised immediately post surgery. A drain was in place and approx 1 cup of bloody discharge occurred a few hours after surgery. The drain was removed 3 days postop. Heavy bloody discharge occurred during the first week. At one week postop check was told a large hematoma was present. The drainage hole is still discharging 5-10ml bloody discharge twice daily (after 2 weeks) and area where drain exited is painful. Is this a concern? When should the discharge stop (told initially it would be a few days after drain removal). Is another surgery warranted? Could there be any underlying pathology causing the complications?

Hello,

Rarely, bleeding and scrotal hematoma formation may occur after hydrocelectomy (less than 5% in our experience). Small hematomas typically resolve spontaneously within 2-3 months. If a large hematoma develops, it may need to be drained to expedite healing.

So, as its only 2weeks since surgery, you can wait for another 2 weeks for It to resolve. There will not be any underlying pathology other than hematoma. In the meantime, if bleeding is heavy with intense pain, you need to consult your urologist.
