Suggest Treatment For Femoral Anteversion And Leg Length Discrepancy

Hello, I am 28 years old and I have Ehler Danlos syndrome (diagnosed 13 years ago). I am dislocating knee at least once a week, my knee turnd in signifanyly. I have been diagnosed with femoral antversion (left hip only with a 47 degree internal rotation compared to 18 degrees on right hip). I have a leg length discrepancy of 2.4cm and my pelvis is tilting (both left side). The surgeon has asked a paediatric surgeon got advice from the information on cat scan and gait analysis. Any advice on treatment as I can only find information for children.

Hi,

The best choice you could have for your issue is a de-rotating osteotomy + an elongation procedure. This would mean two surgeries: One in which your femoral ante version will be brought back to normal, and another one to recover at least 1.5cm of your leg.

By derotating your hip, your knee would be realigned and your patella shouldn't keep on dislocating. Also, by placing your hip on an adequate ante version, you're preventing it from having problems in the future. When the de-rotation has healed, it will be time for you to check out which one of your bones is shorter (whether is the femur or the tibia), and go for an elongation. It will be a long process, but it will be worth it.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Gonzalo Escobar, Orthopedic Surgeon
