Current status evaluation of the kidney with
ultrasound examination is mandatory to decide the final curative pathway of the calculus. Depending upon the size and location of the calculus with functional status of the
urinary tract, various measures can be obtained as:
- Flush therapy (for small, tiny calculus which can be expelled from the urethra, say around size of 4-5 mm)
- Extra corporeal shock wave
lithotripsy (ESWL)
- Intra corporeal shock wave lithotripsy (ISWL)
- Per cutaneous nephrolithotomy (PCNL) for larger stone above 12-15 mm or staghorn
- Dormia basket or Retrograde intrarenal scopy (RIRS) to handle upper ureter and kidney in one set up
- Open nephrolithotomy (where endoscopic interventions not possible or difficult or contraindicated due to various reasons). Intake of anti spasmodic medication as
dicyclomine or
drotaverine will help a lot for pain relief on primary basis.
