Hi, my son have severe grade 3 hydronephrosis, with thinning of the left renal parenchyma, and left ureter is dilated measuring up to 1.9cm in diameter distally. He also have collecting systemon the post void images for the left kidney as well. How severe is this for a 5 month? What can be the preventive care or outcome of this situation? Thanks!!
Gross hydronephrosis with thin renal parenchyma suggests significant obstruction to urine outflow on that side. Since ureter is also dilated, the obstruction is below. This happens in a condition called VUR (vesico ureteric reflux), where the valve between ureter and urinary bladder is incompetent, causing reflux of urine back to ureter and kidney and back pressure.
This may need surgical correction of the valve to prevent progressive kidney damage incase the hydronephrosis is progressing during subsequent scans or he develops urine infection. If he develops fever or crying on micturition, kindly contact your doctor without delay to detect and treat urine infection without any delay
Since the opposite side is normal, chance of renal failure is low.

Suggest Treatment For Grade 3 Hydronephrosis
