Thank you for asking on health care magic.
I would suggest you the following:
1. Suicidal thoughts can sometimes increase during the initial period of treatment with antidepressants but in the long term chances of suicide decrease. You must share your thoughts with your treating doctor.

2. Consider adding anxiolytic to the current treatment at night for improving sleep.
3. Get help in the form of self help cognitive behavioral therapy for depression or professional CBT.
4. If you are not improving for last 5 months you need to change your prescription.
5. Use add on treatments such as relaxation exercises, enhancing Emotional quotient and vitamin D supplement can also help.

Regards
