Suggest Treatment For Low Hemoglobin Count While Having Hemolytic Anemia

My mom was diagnosed hemolactyc anemia a year ago. She has been having to recieve blood transfusions. Just last week they decided to remove her spleen. 5 days later she went back to the hospital because of a fever and severe chest pain. They found a blood clot in her lung that was pretty big. Now her hemoglobin is at 5.5. The hemologists didnt seem to be considered when her blood dropped to 7.1 and less than 12 hours later it dropped to 6.0. 6 hours passed and it dropped to 5.5. The nurses were concerned for her bwcause the hemotologist didnt want to order a transfusion so they had another doctor order it. What can be done for condition? Any advice helps thank you in advance

Hi,

Such rapid blood drop might be due to blood loss somewhere and in her possibility of lung infarction etc as hemolysis can't be such rapid, however with such low levels and suspicion of blood loss, transfusion seem unavoidable though concerns of hematologist might be worsening of hemolysis but keeping an optimal level to avoid stasis of blood was necessary to avoid further compromise in blood flow. Interventional cardiologist or vascular surgeon review for other means of clot removal should have been discussed in this case.
