Hi friend,
The symptoms you described seem to be related to osteoarthritis. It is a very common condition in the elderly population. It is cause by "wear and tear" of the cartilage. Treatments compose of strength training exercise and relieving pain symptom.
It is advisable to continue physical therapy as these have been show to improve daily function and show the progression of the condition.
