Hello. Thanks for asking from HCM.

I can understand your concern. It is good that you are conscious and oriented. The poor concentration, fogginess, neck and shoulder pain could be due to "concussion". It is a type of blunt head injury which is followed by temporary loss of consciousness and loss of memory for the incidence.

Concussion can cause persistence of headache, emotional irritability, confusion for few days. It usually recovers on its own. But sometimes symptoms may persist for few weeks.

To fasten relief, you can follow few tips

: Bed rest
: You can take painkiller like Diclofenac sodium after proper prescription
: Get neck and shoulder massage
: You can consult psychiatrist in case of persistence of your psychological symptoms
: Avoid work which require deep concentration

Hope it will help you. Still no relief, consult doctor. He might advise CT head to rule out any brain injury. Further management will be decided after report.

Thanks. Take care.
