I understand your concern.
You have
ovarian cyst along with free fluid in cul de sac. Cul de sac is a dead end of the amniotic cavity pouch front & behind
uterus. So any inflammation/ infection/ ovulation causing free fluid in
pelvic cavity tends to collect in the Cul De Sac.
Mostly minimal fluid in cul De Sac indicates ovulation ( during routine menstrual cycle) BUt you in addition have some debris & pain along with it.. that indicates [possibility of rupture of an ovarian cyst/ infection in pelvis.)
* Usually in rupture of ovarian cyst pain management along with antibiotics to avoid infection is tried first...
* But in case pain is associated with fever( Sign of pelvic inflammation )- makes an expert consultation along with investigations to find / exclude possible cause of pelvic inflammation & specific treatment.
* About the ovarian cyst you still have in ovary..
- Expectant treatment is first line of management, as many times the cyst resolves on it's own.
-Ovarian cyst which is large / complex cyst/ cysts not resolving after 3 menstrual cycles may need surgery.
Of course, in your case expectant care & preservation of ovaries would be preferred in case of surgery.
