Suggest Treatment For Pain In Lower Abdomen And Night Sweats

Hi, I am experiencing pain and discomfort below navel in abdomen, this has been going on for several weeks and may coincide with coming off HRT after 20 years. I also have terrible night sweats which I assume is menopausal but it happens also when I pass urine. The last few days I have also noticed blood after passing a stool (fresh bright colour) which may well be a separate issue. I am due to have major spinal surgery-with support rods over seven lumbar vertebrae and also had cervical surgery last March. I am worried to proceed with the surgery until I am sure the symptoms I am experiencing at present (including groin pain when walking) is linked to my spine for sure and not a separate issue. Would you suggest I ask my surgeon or GP for a CT scan before going ahead with surgery. I am very confused as to what to do as should have had the surgery some time ago but feel so uncertain- having these new symptoms and keep postponing.

Hello,

* As per my clinical experience, the pain lower abdomen with urinary discomfort indicate possible urinary infection with cystitis.

* The night sweats may be part of estrogen deficiency from weaning off hormone replacement therapy after 20 years.

* The fresh blood in stool after defecation do suggest posterior anal fissure in majority of the cases following severe constipation episode.

My guidelines for primary relief

- dietary precautions to have more of fiber content and bulk forming laxative if hard stool
- anti spasmodic medication as dicyclomine or drotaverine with mefenamic acid
- possible hormone replacement for night sweats
- local application of xylocaine jelly over anal region
- sitz bath twice a day

* Discuss the need of basic lab tests, ultrasound of the abdomen and pelvis with your doc during next scheduled visit.
