Hello welcome to the health care magic

On first impression it seems to be vascular cause like venous thrombosis but no clot present in doppler ultrasound, hence Vascular causes can be excluded here

Rule out filariasis by peripheral smear examination and serological testing

Liver disorder, kidney problems by sgpt, sgot, urea, creatinine estimation etc can be ruled out

EKG done to check cardiac causes

According to cause specific treatment given

Consult good cardiovascular surgeon for examination and discuss your case

Hope this will help you

Take care
