Suggest Treatment For Persistent Back Pain And Fatigue

My husband has had two episodes of severe low back weakness radiating down left leg mostly, but right also. He stands with a cane/walking also/ then his posture is leaning to the left side or backwards. His vitals are stable, no stroke symptoms/. He can raise both arms well, lift legs(when lying down), speaks clearly, no smile deficiency. recent ct scans (dr. states) has old probable strokes. Can you possibly find a diagnosis fo him and some treatment. He has been complaining of severe back pain for sometime now and left leg weakness. Mrs. Alfred Spinks Thank you

General & Family Physician 's  Response
Hello,

This back pain can be due to past strokes and most probably due to neural origin. The description shows clear neural pain. So, I would like to give some tests and medications.

Although the tests are not done, as you ask for a diagnosis, in my opinion, this is Peripheral Neuropathy.

Some of the tests to be done:

1. Electromyography
2. Complete Blood Test including Fasting Blood Glucose.

I would like to give some medication also:

1. Pain killer like Ibuprofen or Tramadol for more severe pain
2. Capsaicin cream for pain or Lidocaine patches
3. Gabapentin 3 times a day for 1 month
4. Vitamin B Complex

Good nutrition, exercise of arms and legs, rest, fluid intake, all has to be maintained properly.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Dipan Samanta, General & Family Physician
