Suggest Treatment For Recurring UTI With Back And Abdominal Pain

I had what they told me was a uti spreading to my kidneys just over a year ago.. symptoms (on and off ever since Feb 2014): back pain, abdominal pain, frequent urge to urinate, inability to urinate, sudden urge to urinate, weakness, fatigue, flu-like muscle pains...I had EXTREME pain when it first occurred last year. It lasted over a month and they ended up inserting a catheter at one time to manually drain my bladder. They gave me at least 5 different antibiotics and kept switching from one to the next as the infection was not responding to the medicines. After they drained the bladder, it finally went away not too long after. But, then it returned (frequent urination)...went away again...and returned..I m now taking a medicine for overactive bladder. An x-ray done last week shows my bladder is moderately distended, but normal in appearance. They ve done a CT scan but the results have not yet come back. They also plan to do a urodynamics test soon. They do not seem to have ANY sense of urgency about the situation-I keep being told chances of permanent damage having occurred from the time when the infection lasted over a month (feb14-Mar14) are very low. I m currently having back pains now-only about a 2 or 3 on pain scale. The pain comes and goes-it always has ever since the infection a year ago. All urinary symptoms are again present and I m weak and having chills often today and my temperature is fluctuating wildly today-as low as 96 and now its sitting at 99....also have weakness and flu-like soreness and lack of energy...I am seriously afraid that whatever it is may be on its way to being permanent or causing worse damage...Do i go to the hospital? My back pain is NOT severe, but the fact that this pain has been present for so long worries me?? Please provide an answer if at all possible?

Hello,

You seem to be suffering from a kidney infection. A few probable causes of your infection are bacteria can travel from the lower urinary tract upwards and infect kidneys, and bacteria elsewhere in the body can travel through blood and infect kidney. Recent catheterization poses increased risk. Low immunity like diabetes is also a risk factor.

Obstruction in the urinary tract is most important risk factor. Kidney infection might be due to acute pyelonephritis, renal and perirenal abscess, emphysematous pyelonephritis, emphysematous cystitis, fungal infections, Xanthogranulomatous pyelonephritis, or papillary necrosis.

If not treated properly it leads to kidney scarring and permanent kidney damage. Please go and see a competent nephrologist or a urologist and get accurate diagnosis and appropriate treatment. You may need tests like complete urine examination, urine culture, blood culture and sensitivity, complete blood picture and kidney function tests.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Pallavi M., Anesthesiologist
