Suggest Treatment For Renal Parenchymal Disease

Dear Sir,
On suggestion by child specialist I have done Sonography of KUB Region of my 8 year old daughter. The sonography report says:-
KIDNEYS: Size RK = 105 x 45mm, LK = 105 X 49MM.
Both kidneys are enlarged, showing diffuse increase of parenchymal echotexture with loss of corticomedullary differentiation. Pelvicalyceal system not dilated. No calculi. No mass. No perinepheie collection.
URETERS: Both ureters not seen dilated.
URINARY BLADDER: Prevoiding study shows normal distention with elar urine. Wall thickness is normal. No intraluminal pathology seen. Postvoiding study shows insignificant minimal residual urine (approx: 5 ml)
Multiple dilated vessels (venous) are seen between spleen and upper pole of left kidney with single varix measuring 36 x 28 mm.
Mild fluid collection present in pelvis.
Impression: Bilateal mild renal enlargement with evidence of? medical renal parenchynal disease.
Multiple tortuous vessels (venous) with single varix (size 36 x 28 mm) located between spleen and upper pole of left kidney -possible spleenorenal porto-systemic collagterals.
Mild pelvis ascites.
Please note that her urine culture shows no infection.
Please suggest whom I will contact.
Regards,
Rajib

Your Ultrasound results of your daughter suggests bilateral medical renal disease. You have to correlate this with the kidney function tests like
urea , creatinine ,gfr etc
It is better you visit a Pediatric nephrologist if you have one in your city otherwise you can go to a nephrologist for further follow up and advice.
