Hi welcome to health care magic...

Your husband is chronic smoker since 30 year and having COPD problem associated with pleural effusion so drain was put for that....

The additionally drug abuse and smoking putting your husband into high risk for mortality with poor five year survival..

He can be admitted to Rehabilitation centre for deaddiction of drugs and smoking...

Regular exercise done and pulmonary rehabilitation will be useful....

He is already on bronchodilator and steroid which needed in such case...

Take care...

Advise : follow advise of your treating pulmonologist and lung transplantation thinking done if possible..

Hope this will help
