Suggest Treatment For The Conditions Mentioned In This MRI Report

What is the treatment for the below mentioned conditions which is as per the latest Mir reports of mine: 1) grade I antero listhesis at L1 S1 level. 2) Diffuse bulge of L 3-L4 disc with right para central inferiority migrated extrusion causing right lateral stenosis with impingement of right L4-L5 nerve roots 3) small disc sequestration adjacent to right lateral recess of L5 vertebrae indenting right L5 nerve root 4) pseudo disc protrusion of L5- S1 with bilateral neural foraminal stenosis causing indentation of bilateral L5 nerve roots I m 51 yrs old and I get pain on and off but not continuous right from the right buttocks uptill the ankle of my right leg. The pain is intermittent and when pain starts,I have to lie down in different positions to get rid of the pain and so far I have not taken any pain killer drugs.

Hi,

It should be L5-S1 level.So you have degenerative spine disease which is causing compression of the nerve roots due to the disc coming out from its normal position. This is causing symptoms of sciatica in you. At present, you need to take rest, use a lumbar corset during, standing, walking, and sitting but not during lying on bed or couch. Take pain-killers and muscle relaxants for 5-7 days and start lumbar extension exercises.

Simultaneously start Gabapentin/Pregabalin with methylcobalamin for 6 weeks minimum along with calcium supplementation. Avoid strenuous activities, forward bending, and lifting heavy weight. If this all fails and your symptoms are not relieved then you might need surgery to stabilize the listhesis (slippage of L5 vertebra over S1 vertebra) using screw and rods along with removal of the disc that is causing symptoms. But I suggest you stick to physiotherapy and medications as most patients respond to them within 6 weeks.
