Hi,

Bipolar type 1 is a serious disorder and requires continuous medication management and regular follow-up with a psychiatrist in order to keep your symptoms in control. Parathyroid problems including both hypo and hyperparathyroidism can lead to the development of neuropsychiatric symptoms. It is absolutely possible that your symptoms are not well controlled on the same medications since you developed parathyroid problems.

It is important to treat insomnia as lack of sleep will further worsen your mood symptoms which is exactly what is happening. Some of the medications that can be tried with your current medications include Zolpidem, Trazodone, Melatonin, Quetiapine. Here are my recommendations:
- See an endocrinologist in order to manage your parathyroid problems
- Work with your psychiatrist to find out the medication combination that works best for your symptoms

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Saralkumar R. Desai, General & Family Physician
