Suggest Ways To Manage Severe Pain Due To Back Issues And Insomnia

I have a very severe back issue, I am under pain management but have not asked my doctor about different pain meds because I have a very high tolerance.
I only take them in the evening to try to sleep because I do not want them to not function if used regular.
Anyway I am on 20 mg of oxycodone and 10 mg cyclobenzaprine and it honestly does not help.
Am I making a mistake by letting my pain get too intense? Also a friend said to possibly try hydromorphone, tries 4 mg did nothing.
Any ideas also for insomnia?

I can understand your concern. Chronic persistent pain usually respond to opioid drugs but these drugs can cause tolerance and have high abuse potential. You have been talking Oxycodone and still the drug is not helping so in my opinion you should talk to your doctor for some better pain medicine. Medicines like SNRI drugs as Duloxetine, Venlafaxine have been found to be very useful in chronic pain conditions when used along with opioids. Other medicines like Flupirtine when taken regularly provides effective pain control. Gabapentine, Pregabalin etc are also other options for chronic persistent pains. Please visit your doctor and discuss about these drugs. Don't ignore your pain as this would increase the suffering only.

For sleep medicines like low dose benzodiazepines or z-drugs can help. But in my opinion control of pain would result in improvement in sleep. Persistent intractable pain is causing the insomnia and its control would help in good sleep.
