Symptoms Of IBS, Ammonia Smelling Loose Stools, Fissure And Itchiness Around Anal Area. Given Zoloft. Problem?

I have started taking 50 mg of Zoloft a day for the last 2/12 and have been experiencing symptoms of IBS the last month. I am currently aware of a small anal fissure - frank blood on the toilet paper after I wipe - and some discomfort and itchiness around the anal area. I am a healthy, 30 something female with no past history of GI upset or problems and can only attribute this to my new medication. I have also started to notice ammonia-smelling,loose stools which are beginning to concern me.

Thanks for posting on HCM,

The correlation between zoloft and the development of your new symptoms could be interpreted directly as the cause of your bowel upset. Zoloft is known to cause a rare side effect known as microscopic colitis which usually occur in patients that are characteristically, though not exclusively, middle-aged females. In order to determine if this is actually what you are facing, you will need to stop zoloft for a while then do a colonoscopy (doctor will request that upon your visit). Colonoscopy is normal or near normal. The changes are often patchy, so multiple colonic biopsies must be taken in order to make the diagnosis. A full colonoscopy is required, as an examination limited to the rectum will miss cases of microscopic colitis. Other diseases such as autoimmune diseases, arthritis etc will have to be ruled out too. Other causes of lower bowel bleeding such as hemorrhoid, anal fissure etc will have to be excluded. Ones cause is determined by your doctor he will establish and appropriate treatment plan.
