The stress sestamibi/rest sestamibi images are markedly abnormal. There is a large perfusion defect involving the inferior and posterior walls extending from the base of the heart out to the inferoapex. The rest images show no evidence of redistribution. This defect is fixed. Additionally, there is a moderate-sized perfusion defect involving the anterolateral wall. This shows partial but incomplete redistribution. The gated images reveal marked dilatation of the LV cavity. The end-systolic volume is 205; normal is less than 70. There is severe generalized hypokinesis more prominent involving the inferior and posterior walls which are essentially akinetic. Overall function is severely depressed. The EF ejection fraction is 21%.
