transabdominal pelvic ultrasound in alady 24 yrs old showed a cystic lesion of about 5x6cm exactly behind uterus in longitudinal scan with diffuse low amplitude echoes inside , transverse scan shows 2small( 3x3) cysts one on each site of the large one behind uterus a small cystic lesion of 2x1.5cm is obviously seen in longitudinal image inside the posterior wall myometrium at lower uterine segment
can you enrich the differential?

Hi this suggests that you have cystic lesion(fluid/secretion filled mass) in the posterior wall and behind the uterus. This may be due to fibroid or due to other cystic lesions. Since the size measuring 5X6 cm and its subserosal (behind uterus) you need opinion from your gynaecologist for its management as they can easily undergo twist causing severe pain/problem.
