Undergoing Surgery Of Recurrent Thyroglossal Cyst

My son who is just 5 years old is suffering from recurrent thyroglossal cyst. He has already undergone two operations of his cyst one at Bokaro and another at CMC Vellore in the year 2009. At the time of 1st operation at Bokaro Steel City, the doctor was quite unaware of the fact that there is a cyst. He had taken the matter very casually and operate the cyst. At the time of operation, he told me that ohho it is a cyst. After that he referred the case of my son at CMC, Vellore. In the month of may, 2009, he underwent a further surgery of his thyroglossal cyst but with no result. Please suggest me that surgery of thyroglossal cyst relates with general surgeon or it requires specialized ENT sugeon. Please also suggest me the important pre-tests required before undergoing surgery of recurrent thyroglossal cyst. Is it curable or not.

Otolaryngologist / ENT Specialist Dr. Sumit Bhatti's  Response
1. The operative procedure is known as Sistrunk's operation.
2. This involves removal of a core of tissue around the tract of the thyroglossal cyst along with a wide margin of normal tissue till the base of the tongue.
3. The middle third of the hyoid bone must be removed to prevent a recurrence.
4. Along with routine pre-op investigations including the fistulogram, remember to get thyroid ultrasound, thyroid function teats and thyroid scan done to locate the thyroid tissue present and to prevent the inadvertent removal of a lingual thyroid, which may be the only functioning thyroid tissue in his body.
5. It is curable. After following the above guidelines, it should not recur.

Pediatrician Dr. Suryakanta Baraha's  Response
thyroglossal cyst is surgical resection, often with simultaneous removal of the midsection of the hyoid bone (Sistrunk procedure), to prevent recurrence. Sistrunk procedure involves excision not only of the cyst but also of the path's tract and branches. A removal of the central portion of the hyoid bone is indicated to ensure complete removal of the tract. It is unlikely that there will be a recurrence after such an operation
