Was Born With Holes In Heart And Was Operated And Had Patched With Teflon. Now Having Difficulties. Advise?

Hello, I need some information regarding my son's open heart surgery, the first one was in 1972 and the second one in 1976 before starting kindergarten. He was born with 3 holes in his heart, one between the left and right ventricle, one between the left and right atria and one between 2 main arteries. He had one hole patched with a Teflon patch. I have not got the specific dates and do not know the surgeon's name. I do have the name of the hospital. My son is having some difficulties with his health and the cardiologist wants information from his surgeries. How can we find out more information for him?
