What Are The Dry Scaly Yellowish Patches On The Center Of The Palm?

Hi, I have this dry, scaly yellow-ish patch of skin on the very center of the palm of my hand. I have tried various lotions, hydrocortizone cream and even Aquafor to see if it would go away. I don't know what it is but it gets cracked to the point where there are small cuts where the creases of my palm are.
