hi doctor , i have a case on my exam and i want u help me solve it please 3 month old infant with left eye watering all the time for the past 2 month , also attacks of uni-lateral redness and yellowish discharge that improved with antibiotic eye drops and reccured after stopping it...what would that be ? ophthalmia neonaturom or dacryocystits ??
in fant of this age definitely have congenital problem of nasolacrimal duct. now nothing can be done except massage over nasal bridge over duct . if it open up then problem is solved, or you have to wait till he grows up. since then it request surgery under GA.
I reviewed history. The history indicates it is congenital dacryocystitis. Ophthalmia neonatorum is bilateral conjunctivitis seen in first month of life. Since age is more than one month and there is history of watering it is congenital dacryocystitis. The treatment is by use of antibiotics and lacrimal massage. If it does not resolve by 6 months then lacrimal probing is required. If probing fails it can be repeated at 1 year. If it also fails dacryocystorhinostony is done after 4 years of age.
