What Caused Stillbirth Due To Nil Heartbeat During Labor?

Fetus had normal heart beat in a routine check up in the afternoon(38 wks 3 days pregnancy). Three hrs later, whe she was taken to Hospital with labor, baby had no heart beat.Still birth resulted - 3 kg baby.
Histopathology of Placenta indicates- Placenta Infarct.

Hallow Dear,

The report placental infarct means the placental blood supply to that area of placenta was jeopardized leading to the death of that area of placenta. Such accident usually occurs:

1. High blood pressure: Pregnancy induced hypertension or essential hypertension.
2. Trauma on the uterus causing separation of the placenta.
3. Diabetes also may cause such infarct.

The growth of the baby appears to be adequate as per the duration of pregnancy. So prolonged cause like pregnancy induced hypertension is less likely cause of such infarct. It causes prolonged diminution of the blood supply to the placenta leading to intrauterine growth retardation. To me, if there is no history of trauma, Diabetes seems to be more likely cause which is known to cause sudden loss of blood. Please ask her about any history of trauma.

In many women, diabetes is latent and becomes overt during stress related conditions like pregnancy. In diabetes, though the baby is premature, it is heavier in weight. Hence combination of pregnancy induced hypertension with diabetes should be thought of.

Please get her glucose tolerance test done. Also, get her blood pressure checked.

Thom this day, start the Folic acid tablets for her which help good development of placenta. During next pregnancy, start monitoring her blood pressure, urine sugar, urine albumin, urinary infection, blood sugar, etc. with the advice of your Obstetrician. Particularly, if in non-pregnant state, no abnormal finding is found, monitoring during the pregnancy is very vital.
