What Causes A Corn With Blister On The Middle Toe?

I have, what thought was, a corn on my middle left toe. It has always been flesh colored and very hard, about the size of a pinky nail. However, it has developed a blister on top of it. Is that normal? I just a head to toe assessment with my dermatologist about a month ago ( all was fine). I forget to mention it, and I think she missed seeing it.

There must be friction point near that toe which is causing the corn. Action may be taken to prevent the friction. Corn cap can be used which will help in reducing or removing the corn. It can be reviewed to observe the progress of corn, its pain or growth.
