What Causes A Painful Bump On The Upper Back?

I have a small spot on my upper back/shoulder area. It looked like a minor bug bite at first but it is not healing. There is not much redness and it is not swollen. The pain is periodic but it is like my skin is tearing. It started about 3 weeks ago.

It could be a small hematoma or other benign swellings like lipoma or fibroma.

If there is diagnostic confusion, a fine needle biopsy might be required.
