What Causes A Painful Sore At The Intergluteal Cleft?

My 45 yr old husband has a painful sore at the top of his butt crack for about a month. It looks like 3 little clear fluid filled blisters surrounded by redness. It hurts mostly after going poop in the morning, by the afternoon he s feeling zero pain. He feels like the pain is around his anus, but there is no redness in that area. He has been monogamous for 5 years, and I, his only partner do not have any STDs.

The narration of issue suggests underlying fissure around anus with secondary bacterial infection causing fluid filled blisters.
Primary helpful tips till you meet his consultant surgeon for curative steps are as following

@Start giving him sitz bath (sitting in a tub filled with lukewarm water 5 minutes, 3 times a day).
@Manage diet to have soft, easy evacuation on regular basis.
@Analgesic as Aleve or else if no history of drug sensitivity.
@Avoid prolong sitting on hard surfaces and long drives.
@Take priority base appointment for getting a prescription of necessary antibiotics and anti inflammatory medicines.
@If possible to get OTC basis, an ointment with Lignocaine will help greatly through local application.
