My wife has a perforated septum. None of the usual reasons for the perforation apply to her. She s not a drug user, nor prior surgeries. No injuries to the nose. Not taking any medication. Randomly her nose starts whistling and there s a small hole. We are scheduled to have it fixed by taking tissue from another part of her nose and covering the hole with it. Sounds pretty simple/straight forward but I worry about doing that without understanding what caused it in the first place. Isn t there a risk that it will just happen again? Shouldn t more emphasis be put on identifying how it happened in the event that there s an underlying issue that needs to be addressed?

Nasal septum perforation has variety of causes. Nasal piercing,long-term topical drug application,decongestants nasal spray,chronic epistaxis, excessive nose picking, rare granulomatous inflammatory conditions like granulomatosis with polyangitis.
now if you have rules out all other causes the i would recommend you to get her workup done for granulomatosis with polyangiatis which is an autoimmune disease effecting the blood vessels. You would need to get urine complete exam along with ANCA levels and chest x-ray done for granulomatosis with polyangitis.
