What Causes A Pus Filled Spot On The Arm Of A Child?

My niece has a huge red spot on her arm. It is very hard all around it and has a open spot in the middle of it. Everytime it gets scratched open it bleeds forever. Puss even comes out of it. She is four, and cries everytime we squeeze it to drain it because it hurts so bad. We put peroxide on it and a bandaid. What is it? Should she go to the ER?
